My theory is it has to be that an NPC TELLS you to go to the Festival/Kill Radhan. This is just a Theory though, a gam- okay stop. I think I got it to trigger by doing Gael Tunnel in First Playthrough, this allows Alexander (Jar Guy) to go to Festival. This is how I did it: teleport to Redmane Castle Plaza. Go up the stairs, and Jerren will be there. Talk to him over and over until he repeats himself. Then, go back and sit at the grace. Now, go back up the stairs, and Jerren will be gone, and the door behind him will be open.
